Information you provide

The quote form asks for your name, phone number, email, state, ZIP code, date of birth, mortgage balance or home value, tobacco use, and whether you have had cancer, heart disease, diabetes, or stroke. Health answers and date of birth are sensitive personal information.

How it is used

Your information is used to respond to your mortgage protection insurance request, review potential coverage options, and contact you by phone or email about that request. A quote request is not an application for insurance and does not establish coverage.

Service providers and security

When you submit a quote request, your answers are securely transmitted to GoHighLevel, our customer relationship management service, to process your request. This website does not store your quiz answers in browser storage or a separate lead database. Unsubmitted answers remain in memory while the form is open. A temporary confirmation marker contains no quote answers.

Your choices

Completing the form is voluntary. You may close the form without submitting it. By selecting Request my quote, you consent to using the information you provide, including health answers, to review your request, and to contact by Vitality Key Financial by phone and email. Consent is not a condition of buying insurance. This form does not request consent to automated marketing texts or prerecorded calls.

Privacy requests

You may request corrections or ask to stop further contact by replying to Vitality Key Financial when the team responds to your quote request. Vitality Key Financial must confirm its official privacy contact and applicable retention practices before collecting live requests.
